Slight correction: Teller did the coin vanish, and then the surgeon pulled the coin out from Penn’s “brain” (according to Penn). lol. He indicates that the surgeon was cutting into his skull while he was awake. It’s amazing that they thought they could show that on TV.

That being said, I was amazed when I heard Penn slamming David Blaine on a podcast for David’s recent “real bullet-in-the-mouth” trick, saying that he had legitimately lost his mind and that it was so dangerous that it couldn’t be called “magic”.
